Grant Application Procedure

Grant Application Procedure for First Time Applicants

Once you have determined that your request falls within the established guidelines of the Foundation, you may submit a request for support. All proposals must include the components listed on the Proposal Check List below and must be submitted electronically as Word documents unless noted otherwise. If you have documents that you are not able to send electronically or would prefer us to see them in a paper version, you are welcome to mail them. Proposals will not be reviewed unless they are complete. All requests must be accompanied by a completed Proposal Submission Form, which can be downloaded off the website.

Proposal Check List (for first time applicants)

  • A cover letter providing the name and a brief description of the proposed program/project for which funding is requested, a general statement about what the proposed program will achieve, and the amount requested for one year.

  • A description of the organization including its mission, history and capacity to manage the proposed program/project.

  • A full description of the program/project for which funding is requested including purpose, need(s) to be addressed, population to be served, brief bios of key organizational and/or program/project staff, program goals/objectives and how they will be measured.

  • A brief explanation of what will be different as a result of your work. How do you define success and what will it look like? If the proposed program/project is part of a larger effort whose long term impact may not be realized by the end of the grant period, please explain and describe the short term outcomes or benchmarks you expect to achieve within the grant period.

  • Budgets showing revenue and expenses for the proposed program/project and for the organization. Budgets should cover the year in which funds will be used AND the year immediately preceding that year. Beginning and end dates for fiscal years should be clearly stated. (May be submitted as a PDF)

  • Current sources of support and the fundraising plan as it relates to the proposed program. For foundation funding, please include foundation names, amounts, and whether funding has been received, committed or is pending.

  • A list of the organization’s Board of Directors provided on a separate page with names and affiliations. Also, please note if all board members contribute financially to the organization (“100% participation”) and if not, why not. (May be submitted as a PDF)

  • Please share a management challenge that the organization is currently facing and how it is being addressed (or describe a management challenge the organization faced in the recent past and how it was dealt with).

  • Documentation of the organization's tax exempt status (may be submitted as a PDF)

  • Most recent annual report and/or other materials that may be of interest to the Foundation e.g. articles, photos, letters of support, client profiles/case studies (may be submitted as a PDF)

  • A completed Proposal Submission Form

Grant Application Procedure for Requests for Continued Funding

In an effort to streamline requests for continued support, please submit those items listed in the Proposal Check List electronically in Word documents unless noted otherwise. Additional information such as most recent Annual Report, articles, photos, letters of support, client profiles/case studies are welcome but not required. If you have documents that you are not able to send electronically or would prefer us to see them in a paper version, you are welcome to mail them.

While continued funding cannot be guaranteed, requests are due by the deadline noted in your grant report form unless other arrangements are made in advance. Incomplete requests and/or requests not received by the deadline risk being pushed back to the next funding cycle.

  • Goals and objectives for the coming year and how they will be measured.

  • Bios of key organizational and/or project staff.

  • Budgets showing revenue and expenses for the program/project and for the organization for the year in which the grant funds will be used (may be submitted in Excel).

  • Current sources of support (received, committed and pending) and fundraising strategies as they apply to the proposed program.

  • Please share a management challenge that the organization is currently facing and how it is being addressed (or describe a management challenge the organization faced in the recent past and how it was addressed).

  • A list of the organization’s Board of Directors (may be submitted as a PDF).

  • Documentation of the organization’s tax-exempt status only if it has changed in the last 12 months (may be submitted as a PDF)

  • Grant report

  • A completed Proposal Submission Form
